Cleanup use of QT_VERSION in preprocessor directives

The Qt5 modules contain a few dozen preprocessor directives mentioning QT_VERSION. These should be reviewed and cleaned up, as most of them appear to be irrelevant for Qt5, particularly as it is not intended that individual Qt5 source files can be compiled against Qt4.
