Add support for TLS extensions (RFC 5246)

Provide a generic mechanism for setting and retrieving TLS extensions in the client hello and server hello provided during the handshake process.
In some cases, the extensions included in the server hello are dependent on what is sent in the client hello, so applications must have access to client hello extensions when generating the server hello extensions.
