want more fine-grained control over reviewed-by footers

as a follow-up to QTQAINFRA-181: it would be useful if one could set different footer names for different scores. specifically, CRVW=2 => Approved-by, CRVW=1 => Reviewed-by, and possibly CRVW=-1 => Contested-by.
